Helena Dimitrakoudis MD, CCFP

Dr. Dimitrakoudis holds an Honours Bachelor of Science degree in Physiology from McGill University and an MD degree from the University of Toronto Medical School, graduating with Honours in 1996.

She completed her residency program in Family Medicine at the University of Ottawa, and worked in Ottawa for several years while starting her family with her husband, while he completed his Neurology residency program. After moving back home to Toronto, she opened her own family medicine practice in the Kingsway area and later in Roncesvalles Village. Most recently her practice has been located at St. Joseph’s Hospital. Over the years, Dr Dimitrakoudis’ practice has transitioned from mixed family medicine and walk-in medicine to focusing on comprehensive family medicine, caring for families, from newborns to the elderly, with a strong focus on evidence-based medicine and preventative care.  Dr. Dimitrakoudis has many years of experience in family medicine, knows the patients in her practice well, and manages and coordinates all of their medical care. Dr. Dimitrakoudis enjoys the vast diversity and diagnostic challenges that family medicine provides and takes pride in doing her best to keep her patients healthy. 

Jeremy D’Souza MD, CCFP

Dr. D'Souza's practice is focused on patient-centered medical care, following evidence-based guidelines, and making time for patient counseling and discussion. He uses shared decision-making while ensuring patients have what is reasonable and appropriate for them to remain healthy. Outside of work, he enjoys playing guitar, cooking, chess, woodworking and weightlifting, among other hobbies. He speaks English and French. He likes having conversations with people about their health, and he makes longer appointments, when possible, to accommodate a patient’s needs. He welcomes the ongoing challenges his career presents and is grateful for the privilege to serve his community with full-spectrum outpatient primary care. He encourages patients to reach out regarding anything, including issues pertaining to underrepresented areas of family medicine such as women's health, mental health and pediatrics.


Dr. D'Souza is local to Etobicoke and, barring the unforeseen, he currently plans on practicing in the area for the next 30 years. At work, he particularly enjoys listening to a patient's problem and, if necessary, turning over every stone until he finds a way to be a part of the solution.

Dr. D'Souza studied medicine at the University of Medicine and the Health Sciences in St. Kitts, and completed his residency training at the University of Pittsburgh Medical Center Family Medicine Residency in McKeesport, Pennsylvania. Prior to joining us, he has had about 3 years of practice experience in the GTA. He holds an Unrestricted Certificate of Registration to practice in Ontario and he holds a US national practice license under the American Board of Family Medicine, with individual state licenses to practice in Michigan, Ohio and Pennsylvania.

Sarah Nadeem MD, CCFP

Dr Sarah Nadeem is a Family physician who finished medical school in Karachi, Pakistan in 2009. She then obtained her MSc. In Clinical Dermatology at Cardiff University, UK in 2012. Dr Nadeem then migrated to Canada and obtained MCC certifications and furthered her Canadian training in Family Medicine, where she was Chief Resident at the University of Western Ontario.

Dr Nadeem has a broad range of clinical interests in addition to primary care including; Clinical Dermatology / Aesthetic Medicine, Women’s Health / Immigrant Health, and community medicine. She has over ten years of experience in primary care and prides herself in being a health advocate for her patients and community.

Dr Nadeem is fluent in English and Urdu. She spends her spare time with her husband and daughter and enjoys mentoring International Medical Graduates.

Lloyd Mai MD, CCFP

Dr Mai is a family physician who was born and raised in Ottawa, Ontario. He obtained his undergraduate degree in science with specialization in Biochemistry at the University of Ottawa before moving to Toronto, where he completed medical school and Family Medicine residency training at the University of Toronto.

Dr Mai provides comprehensive care using evidence-based medicine for patients of all ages. Outside of medicine, he is an avid chess player earning the title of National Master by the Chess Federation of Canada.

Kevin Ho MD, CCFP

Dr Kevin Ho attained his medical degree from the University of Sydney in 2017. He then returned to Toronto to complete his Family Medicine residency with the University of Toronto. Dr Ho provides comprehensive, high quality care to his patients.

Jon Pinto MD, FRCPC

Dr. Jonathan Pinto is a board-certified Internal Medicine Specialist Physician who practices Internal Medicine and Adult Primary Care. He holds an undergraduate degree in Computer Engineering from the University of Toronto and attended medical school at St. Matthew's University, after which he completed his residency in Internal Medicine in York, Pennsylvania. Dr. Pinto has practiced as an Internal Medicine physician at Rockledge Regional Medical Centre in Florida for 5 years and has extensive experience in acute and chronic care management.

Dr Pinto is board-certified by the American Board of Internal Medicine (ABIM) and participates in the Maintenance of Certification.

Abhishek Seth MD, CCFP

Dr Seth completed his residency training with Drexel University in Philadelphia, PA in a Family Medicine residency program that emphasized evidence based primary, hospital, and geriatric medicine. This program worked with primarily under-served populations with unique medical challenges. Dr Seth held the role of Chief Resident during his final year with the program.

After completion of residency, Dr Seth attended Weill Cornell’s Geriatric Medicine Fellowship working at one of the largest hospital systems in the United States located in Manhattan, NY. This program allowed him to further his interest and garner his skills in the care of the elderly.

Since returning to the GTA in 2019, Dr Seth has worked in all areas of Family Medicine. He has a keen interest in hospital inpatient medicine and is a clinical associate with Trillium Health Partners, and serves as the Chief of Hospitalist Medicine with Headwaters Hospital. Dr Seth has a small family practice with a focus on geriatric patients with complex care needs.
